owow Dental needs your help in protecting your personal and medical information. Part of our responsibility is to adhere to HIPPA policies and also share how we may or may not collect, use, and share personal information about you, both online and offline. Your responsibility is to be aware of our Privacy Policy and use common sense and extreme when communicating with us using a digital form. DO NOT SEND PERSONAL INFORMATION VIA EMAIL OR WEBSITE FORM!

Our Privacy Policy applies to all patients and users of Now Dental services and website. This privacy policy DOES NOT COVER platforms such as Facebook and Instagram. In those cases, their existing Privacy Policies govern the collection, use of, and sharing of information.

We collect information from you when you use the Now Dental website where you may be asked to enter personal information such as name, email, phone, etc. However, you may visit our website anonymously. The information may be collected via mobile or other electronic devices.

Although we do our best to protect your personal information, we cannot guarantee the security of your personal information transmitted to our Website. You are welcome to confirm your dental appointment texts by phone.

Therefore Now Dental will never request personal or medical data to be sent via text, email, or form hosted on our website.

If you receive any text or email request that appears to come from Now Dental, which asks for billing, health, or personal information, do not respond or click any link. Instead, please call us to verify any text, email, or SMS message.

You have the right to receive an account listing instances in which we or our business associates disclosed your health information for purposes other than treatment, payment, healthcare operations and certain other activities for the last 6 years. Requesting this accounting more than once in a 12-month period, may incur a reasonable, cost-based fee for responding to these additional requests.

Notifying Insurance Companies of Payment
When you pay in full outside of your insurance plan for services you may request that we restrict this information and not disclose it to your healthcare plan or insurer.

You have the right to request that we amend your health information. This request must be in writing and it must explain what information and why the information should be amended. We have the right and in some cases may be legally obligated to deny your request under certain circumstances.

Now Dental may collect demographic information, such as your date of birth, gender, ethnicity, and primary language; your disability status and military/veteran status. If you apply for financing, we may collect social security identifiers, current and former employers, current and former addresses, and other information related to your finances. In your patient forms, we may collect marital status if you provide contact information for your spouse. Other information related to the medical/dental care we provide collected includes; previous illness history, existing illness history, HIV/AIDS status, pregnancy status, and recent travel history.

We collect this information directly from you, a parent, your guardian or responsible party.

When do we collect it
We collect this information from you when we create or update a patient account record or record patient treatment for an authorized medical partner., contacting us, applying for a job, applying for financing, or submitting patient forms. We may also receive this information from vendors who collect it on our behalf.

The purpose for collecting it
We use this information to ensure patient safety, process insurance claims, and provide customer service. We may use this information for other similar purposes related to the operation of our business, or as we may notify you from time to time.

Medical Information and Health Insurance Information
What we collect
Now Dental collects information about your current and past dental and overall health, current temperature, dentist/physician visit history, allergies, medications, and specific medical diagnoses. We collect dental or health insurance information.

We collect this information directly from you, your guardian or responsible party when you submit patient forms.

Now Dental uses this information to ensure patient safety, provide dental services, process insurance claims, provide customer support and service, as well as detect and investigate insurance fraud or other illegal activity.

If you apply for a job with us Now Dental, we will collect any information you submit about previous employers, work experience, your resume, or other information you provide with your job application.

From whom we collect it
We collect this information directly from you. We may also receive this information from a social network if you choose to have it imported from the social network.

The purpose for collecting it
We use this information to evaluate your suitability for employment by Now Dental.
